Meckel's Diverticulum

Today’s MRCS Knowledge short notes are on Meckel’s Diverticulum.


Background

Ileal remnant of the vitellointestinal duct
Contains ectropic gastric or pancreatic mucosa



Aetiology

Rule of 2s:
• 2% population affected
• 2 inches long
• 2 feet proximal to ileocaecal valve (on anti-mesenteric border)
• 2% are symptomatic
• 2 years of age at presentation


Signs and Symptoms

• Most are asymptomatic
• Severe rectal bleeding
• May present as intussusception/volvulus or diverticulitis

Investigations

  • Technetium scan: increased uptake by ectropic gastric mucosa in 70% of cases

Management

Open or laparoscopic surgical resection in symptomatic cases.


Prognosis

Complications: haemorrhage, obstruction, diverticulitis
